Please submit the following form for EACH of your children who will be on campus today. This should be submitted by 7:15 a.m.

IMPORTANT: If your child is not going to be on campus for all or part of the day, for ANY reason (e.g., appointment, illness, out of town, quarantining, isolating), you must immediately provide details to the School by calling (315) 446-2452 or emailing Attendance@mphschool.org. Timely attendance information is critically important to safeguard the health and well-being of the whole MPH community during the pandemic.
